B-5 Five Year Proposed Construction Plan

To be signed by the Secretary Treasurer after School Board approval and submitted to the Facilities Branch. Note:  All of these forms are to be used in sequence by the boards.

This form will inform the Facilities Branch of the division’s long term facility plans for their division.  It is to be submitted annually.

B-1 Application to Proceed with Preliminary Project Planning

To be signed by the Secretary Treasurer after School Board approval and submitted to the Regional Director of Education, who will then sign the form and submit it to the Facilities Branch.

This form is to be submitted for every capital request that the division wishes to be considered for provincial funding, either in the current fiscal year or in the year immediately following.

B-3 Application for Approval of Tender Award and Funding

To be signed by the Secretary Treasurer after School Board approval and submitted to and submitted directly to the Facilities Branch.

After the division receives the Ministry of Education E-2 approval for their capital project, they will complete drawings and specifications and submit them for ministry approval.  Once the drawings are approved by the ministry, the division may tender the project.

The division will submit the tender results along with the completed B-3 application for ministry review and approval.

CCA - Confirmation of Contract Award

To be signed by the Secretary Treasurer and submitted directly to the Facilities Branch.

After the division receives the E-4, they may sign the construction contract and submit the Confirmation of Contract Award (CCA).

Upon receipt of the CCA, if the value of the ministry share of the approved project cost exceeds $50,000, then the ministry will release the first half of its share to the board.  The remaining ministry funds will be released upon receipt of the Form C-1 Certificate of Substantial Completion.  If the ministry share is less than $50,000, the entire share will be released to the board upon receipt of the C-1 Certificate of Substantial Completion.

C-1 Certificate of Substantial Completion Form

This form is to be used only if the contractor or prime consultant does not provide their own Substantial Completion Certificate.

To be completed by the contractor and consultant and submitted to the owner for signature.  A copy will then be forwarded to the Facilities Branch.  Upon receipt of the C-1, the ministry will release any remaining ministry funds for the project.
